Enrollment Procedure

The following is a general guideline to the steps of enrollment at DayStay:

  • Interested individuals can obtain enrollment forms by visiting the center (135 Candler School Road, Candler, NC 28715) during our hours of operation, 7:30 a.m. to 5:30 p.m., by calling (828) 665-1575, or by emailing daystaymiller@bellsouth.net.

  • The enrollment packet includes an application and medical forms.  A medical examination must be completed by the participant’s physician. All application forms must be completed for review by DayStay prior to the participant’s first day of enrollment.

  • A visit by the participant and caregiver is encouraged.  This visit will allow the applicant to take part in the program and will help the staff to determine the applicant’s needs and interests.

  • During the visit, a DayStay staff member will discuss how well the applicant did during the visit, go over the remaining paperwork, and discuss payment/possible financial assistance.

  • We will then set-up a care schedule or place applicant on a waiting list, if necessary.

Attendance
Full time enrollment is considered 5 days per week; however, participants may elect to attend fewer days per week (as space allows).  Participants scheduled days must be selected at the time of enrollment.  Staffing is based on the scheduled daily attendance.

If participants will be arriving after 10:00 a.m., caregivers are asked to call to ensure we have enough lunch prepared and maintain proper staff ratio.

Participants with more than 10 consecutive absences will be removed from services, but may be allowed to re-enroll when space is available.
